Vigabatrin cost and coverage

On this page

Short answer: what you pay for vigabatrin depends on which product is prescribed, your insurance plan, and the pharmacy. We do not publish prices, because list prices change and what each family pays differs. What we can explain is how coverage works, what to ask for, and how long each package lasts at a given dose.

Why we show no prices

Prices for specialty medicines vary by plan, pharmacy and program, and change over time. A number on this page would be wrong for most readers. Ask the dispensing pharmacy for your actual cost before the first fill, and again if the product or dose changes.

How vigabatrin is dispensed

Vigabatrin is available only through the Vigabatrin REMS, a safety program for the vision-loss risk. Prescribers must be certified, patients enrolled, and pharmacies certified.1,2 In interviews, patients and caregivers said the REMS reassured them but also created access burdens, such as travel to certified prescribers or pharmacies.3 See REMS.

Insurance steps to expect

  • Prior authorization. Many plans ask the prescriber to justify a specialty medicine before they cover it. Ask the prescriber's office whether it has been sent.
  • Product choice. A plan may prefer a generic powder or tablet, or a particular brand. If your child's prescriber chose a specific form for a reason, such as the ready-to-use liquid for an infant, the office can ask the plan for an exception.
  • Timing. For infantile spasms the first days of treatment matter.4 Tell the pharmacy and prescriber if coverage is holding up the first fill.

Copay and assistance programs

Many specialty medicines have manufacturer copay or patient assistance programs, and rules differ by brand, generic and insurance type. Ask the dispensing pharmacy, or the manufacturer of the specific product you were prescribed, whether a program applies to you. This site is independent and does not run or endorse any program. See brands for which company holds each product.

Brand and generic

Generic vigabatrin powder packets and 500 mg tablets are made by several companies. The only ready-to-use liquid is the brand Vigafyde.5 The liquid is labeled only for infantile spasms in babies 1 month to 2 years old.1 Plans may cover these products differently, so the cheaper option on paper is not always the cheaper option for you.

How long a package lasts

Comparing price per mg can mislead, because each form is used differently. Here is how much vigabatrin each package holds, from the labels:

PackageContentsTotal vigabatrin
Ready-to-use liquid (Vigafyde)One 150 mL bottle at 100 mg/mL15,000 mg
Powder for oral solution50 packets of 500 mg25,000 mg
Tablets100 tablets of 500 mg50,000 mg

From the FDA labels.1,6,7

The powder labels say each dose is mixed fresh from whole packets and the rest thrown away. A 125 mg dose still uses a full 500 mg packet.6 An opened liquid bottle is measured by the mL and thrown away 90 days after opening.1

Dose, twice a dayLiquid: one bottle lastsPowder: one carton lasts
125 mg (5 kg baby, starting dose)60 days25 days (2 packets a day)
375 mg (5 kg baby, maximum dose)20 days25 days (2 packets a day)
750 mg (10 kg baby, maximum dose)10 days12 to 13 days (4 packets a day)

Arithmetic from the label package sizes and dosing tables. Infant doses change with weight and titration, so these are snapshots.1,6

For an adult on the recommended 1,500 mg twice a day, a bottle of 100 tablets lasts about 16 days (6 tablets a day).7

The point is not that one form is cheaper. It is that switching forms changes how many packages are dispensed each month, which can change what the plan approves and what you pay. When a prescriber changes the form, ask the pharmacy to re-check coverage and quantity limits. Switching also changes the number of mL per dose; see liquid vs powder and the dose calculator.

What to ask the pharmacy

  • What will I pay for this product each month, after insurance?
  • Is a generic or a different form covered at a lower cost, and is it right for my child's prescription?
  • Is there a copay or assistance program for this product?
  • How many days does each fill cover at the current dose?

Whatever the form or brand, the medicine and its risks are the same, including the risk of vision loss. For the full list of products, see brands; for common questions, see the FAQ.

Common questions

How much does vigabatrin cost?

It depends on the product, the pharmacy and your insurance plan, and list prices change. We do not publish prices for that reason. Your pharmacy can tell you what you will pay for the product your prescriber chose.

Is there a vigabatrin copay card or coupon?

Copay and assistance programs exist for many specialty medicines. Ask the dispensing pharmacy, or the manufacturer of the specific product you were prescribed, whether a program applies to you and your plan. Programs differ between brands and generics.

Can any pharmacy fill a vigabatrin prescription?

No. Vigabatrin is dispensed only through pharmacies certified in the Vigabatrin REMS, and the patient must be enrolled. Your prescriber or insurer can point you to a certified pharmacy.

Is the ready-to-use liquid cheaper than powder?

There is no single answer. What you pay depends on your plan's coverage of each product. The amount used also differs: powder doses use whole packets and throw away the rest, while a liquid bottle is measured by the mL and kept up to 90 days after opening.
